How to get In-Home Services for Youth

Rebuild Therapy and Treatment Services LLC is a Medicaid approved provider. In order to get intensive in home therapy services, a legal guardian would need to call PerformCare at 877-652-7624.

Initial Steps

PerformCare will direct the call to the most clinically appropriate place. If Children’s Mobile Response is recommended, a trained case manager discusses concerns and makes a recommendation based on the needs of the family.

Building Your Team

Case managers have a provider’s list that they utilize for intensive in home services, which Rebuild Therapy LLC is included on. Regardless of the case manager choosing Rebuild Therapy LLC, we want the focus to remain on families getting the mental health services needed. It should be noted that Children’s Mobile Response provides 8 weeks of stabilization services free of charge to families.
